Assumption of Risk, Release of Liability & Waiver
Bunker Hill Karate and Fitness, LLC d/b/a Prospect Martial Arts (“PMA”)
I (or the parent/legal guardian of the minor named below) understand that martial arts training is physically demanding and involves inherent risks of injury. These risks include muscle strains and sprains, bruises, joint injuries (knee, shoulder, elbow, wrist, hip, ankle), impact injuries to the head, face or body, fractures, ligament and tendon injuries, and in rare cases concussion, spinal injury or other serious harm. These risks exist during warm-ups, technique practice, drills, sparring, testing and use of the facilities, regardless of age, fitness level or experience.
I represent that I (or the minor) am physically and mentally able to participate. PMA recommends consulting a physician before beginning any new physical activity.
Assumption of Risk
I voluntarily assume all risks of injury, illness or death arising from participation in PMA programs or use of PMA facilities, including risks caused by the ordinary negligence of PMA, its owners, instructors, employees or agents.
Release of Liability
To the fullest extent permitted by Connecticut law, I release, waive and discharge PMA, its owners, officers, instructors, employees, volunteers and agents from any and all claims arising out of or related to participation in PMA programs or use of its facilities, including claims arising from the ordinary negligence of PMA or its agents. This release does not apply to injuries caused by the intentional misconduct or willful/wanton acts of PMA.
I agree to follow all PMA rules and codes of conduct. Non-compliance may result in suspension or termination of membership without refund for unused services.
Photo/Video Consent
I consent to PMA’s use of photos and videos of me (or the minor) for promotional purposes (website, social media, print materials).
This agreement is governed by Connecticut law. Any dispute shall be resolved in the courts serving Prospect, Connecticut.
I have read this document, understand it is legally binding, and have had the opportunity to ask questions. I am signing it voluntarily. A copy is available at any time through my student portal.
